As Fifth Hokage, Tsunade spends Shippuden balancing the instincts of a legendary fighter with the responsibility of sending other people into danger. She is one of the Sannin, the greatest medical ninja of her era and Sakura's teacher, capable of turning perfect chakra control into both extraordinary healing and devastating physical strength. Her Creation Rebirth technique lets her survive wounds that would kill almost anyone else, but her most important decisions often happen before a fight begins. Tsunade repeatedly backs Naruto when older leaders see only the Nine-Tails or an Akatsuki target. During Pain's assault she uses Katsuyu to protect and heal the village on a massive scale, placing the people of Konoha ahead of her own safety exactly as a Hokage should.

Naruto's first completed Rasenshuriken is powerful enough to destroy Kakuzu's chakra network at a microscopic level, but the same close-range contact damages Naruto's arm. Tsunade warns him against using it again because continued exposure could permanently ruin his ability to mold chakra. Sage Mode later lets Naruto throw the technique instead of holding it through the impact.
What changed
The technique does not become safer because its destructive effect is reduced. Naruto changes how it is delivered so the part that made it unacceptable to his own body is no longer attached to him at detonation.
